Testicle cuffs
The one piece of gear in this family whose point is not in the sensation but in the closure.
What are testicle cuffs?
Testicle cuffs are ring-shaped closures that go around the scrotum, between the body and the testicles. Once shut, the testicles can no longer slip back through them. A common version consists of two linked rings, one around the scrotum and one around the base of the penis; a plain padlock is used for it too.

Why this is not a pain tool
The rest of the CBT family does something. The parachute hangs weight, the ball crusher presses, the ballstretcher keeps distance, the humbler locks a posture in place. Testicle cuffs do none of that. They close, and that is where it stops.
That is exactly the point too. The encyclopedia describes the use in terms of ownership: one person enjoys the feeling of being owned, the other the feeling of owning someone, and wearing these rings makes visible that the genitals belong to the partner. That puts this object closer to the collar than to the gear it sits among.
The word says the same. Etymonline has cuff start as “bottom of a sleeve”, mid-fourteenth century, with the meaning “a fetter for the wrist” only from the 1660s. A cuff is at heart a piece of clothing at the end of a limb. You do not call something like that an instrument of torture; it is something you keep on.
What to watch for
The spot where this ring closes is anatomically the busiest of the whole area. The StatPearls entry on the testes describes that the spermatic cord carries the top of the testicle, and that the testicular arteries run from the abdomen through the deep inguinal ring and join that cord there, together with the artery of the vas deferens and the cremasteric artery; the drainage goes by way of the pampiniform plexus. So everything that separates the testicles from the body closes around the bundle that their blood supply runs through.
In practice that means three things. Choose a size that is not tight but only keeps the testicles from slipping back. Agree on a time, because this is gear you forget precisely because it does not hurt. And make sure the lock can open without a tool, or with a tool that is within reach: a missing key is, with a padlock, not an inconvenience but the whole problem.
