Cock ring
The most innocent-looking piece of gear in the toy box, and the only one that comes with a clock.
What is a cock ring?
A cock ring is a ring around the base of the penis, sometimes including the scrotum, that slows the drainage of blood from the erection and so holds the erection longer. In Dutch it is called a penisring. It sits at the edge of CBT, but it gets used just as often outside BDSM.

How it works
The ring does nothing active. It lets blood in and holds it back on the way out, and that is enough for a firmer, longer-lasting erection. For erection problems the same principle gets used medically. First a vacuum pump to bring on the erection, then the ring to hold it.
For sensation the material matters a lot. Stretchy silicone models come off under any circumstances. Rigid steel rings sit tighter and are the hardest to remove once swelling sets in.
What to watch for
Here the literature is unusually clear, so no rules of thumb from hearsay.
The encyclopedia holds that a ring should not be worn for longer than about thirty minutes. The first sign something is going wrong is a penis that turns numb, painful, or cold. Wrong use can cause priapism, an emergency that can lead to serious and lasting damage if left untreated. Falling asleep with a ring on is the one scenario that gets explicitly warned against, because it can cause temporary or permanent nerve damage.
What happens when it goes wrong is in the urological literature. Noegroho and colleagues describe in International Journal of Surgery Case Reports how strangulation runs its course, from reversible congestion and edema, through loss of sensation, to permanent tissue damage. Koifman and colleagues stress in International Braz J Urol the one thing that matters then. The object has to come off right away, and that is a job for the emergency room and not for a pair of pliers.
In practice, set a clock, when in doubt choose stretchy over rigid, and if a ring will not come off, go to the emergency room right away.
What I make of it
Of all the gear in the toy box, this is the piece people underestimate most, precisely because it looks so innocent. A ring, that is all it seems to be.
What I always tell women who are curious about it is that the clock decides everything here. The good feeling and the moment it goes wrong sit closer together than you would think, and you do not feel the edge coming. That is why I go for a stretchy model that comes off under any circumstances, set a timer, and agree beforehand that it comes off the moment anything turns cold, numb, or painful. If you want to try it at your own pace with someone who keeps an eye on the time, that is exactly the kind of thing we can take slowly on a date.
