Glossary
The oldest position in the whole genre, and the only one where the giver feels exactly what is happening.
Over-the-knee (abbreviated OTK) is the position where the receiver lies belly-down across the giver’s thigh with the buttocks raised. Over-the-knee is not an act but a position: what happens is spanking. The giver sits, the receiver lies, and the free hand holds on.

It is the image almost everyone pictures when they hear spanking, and not by accident.
The usual form: the giver sits on the edge of a chair with the feet flat on the floor. The receiver lies across, upper body and legs down, hips at the highest point. The giver’s hand rests on the lower back or takes a wrist; that hand does more work than the striking hand, because it decides whether someone stays put. Then the warm-up begins.
The image is so familiar because the act is common. Jennifer Rehor surveyed 1,580 women from the kink community about 62 BDSM acts and published in Archives of Sexual Behavior that spanking had been done or received by 93.99 percent of them. Only touching and kissing scored higher.

The StatPearls monograph on the gluteus maximus describes that muscle as thick and fleshy, quadrangular in shape, running at about 45 degrees across the buttock to its attachment, and as the largest and most superficial of the gluteal muscles. It is exactly that mass of muscle that bulges in the over-the-knee position, and that is why the position is so old: it presents the thickest tissue and keeps the bones around it out of the way.
What is not just under the surface is worth knowing too. The sciatic nerve, according to the same series the largest nerve in the body, leaves the pelvis through the greater sciatic foramen beneath the piriformis muscle and runs down the back of the thigh, deep under the long head of the biceps femoris. That nerve lies under muscle tissue. The real risk zones are the hard points: the tailbone and the hip bones, exactly the two spots the encyclopedia entry on erotic spanking also flags.

The appeal is largely the asymmetry: one person sits upright and clothed, the other lies down. That difference in height says without words how the roles are split.
Over-the-knee is not the mildest position, only the most domestic. What I see myself is that after about fifteen minutes it starts to pinch, because the head hangs low and the thigh presses into the belly. With large differences in body size the position works poorly anyway.
There are three variants, and the difference is in what the legs do.
The plain form is over one thigh, with the other leg relaxed. Anyone who wants more control locks the receiver’s legs between their own ankles: the leg lock, the strictest form. The third is the variant on a bed, with the legs stretched out, more comfortable for longer sessions.
What sets the careful version apart: the giver picks a seat height where their own back stays straight. Most over-the-knee sessions end not because the receiver can’t take it anymore, but because the giver cramps up.
“Over-the-knee is a boot.” In fashion it is, and that makes this word hard to search. The encyclopedia describes over-the-knee boots as long boots that fully or partly cover the knee, originally men’s riding boots from the fifteenth century, with the same abbreviation OTK. Only there it means above the knee, and here it means over someone’s knee.
“Over-the-knee is the childish variant.” The position comes from the domestic sphere, but says nothing about the intensity. With a hairbrush or a small paddle the same position gets fairly heavy, and the build-up decides how far it goes, not the choice of furniture.
