Prostate massage

The same maneuver appears in a urology textbook and in a sex guide, and that produces two completely different expectations.

What is prostate massage?

Prostate massage is applying pressure to the prostate with a finger or toy, usually through the front wall of the rectum. The act exists in two worlds that have little to do with each other: urology used it to draw off gland fluid for testing, and in the bedroom it is about what the pressure makes noticeable.

What happens during a prostate massage

Pressure is put with a finger or a curved toy on a spot that lies about a finger-joint deep behind the front wall of the rectum, and that pressure is held or repeated in a slow rhythm.

The feeling that comes with it is rarely sharp. People describe more of a dull, broad urge that resembles needing to pee and only tips over after a minute or so. That is also why a prostate massage almost always runs aground on haste: whoever wants to see a reaction in ten seconds is pressing too hard on the wrong spot.

A recognizable pattern for a couple trying it for the first time: first a quarter of an hour with no penetration at all, then one finger with plenty of lube, then a few minutes searching for the spot that feels different from the rest, and nothing more. There need be no orgasm at the end, and with a first time there usually is not one.

Where prostate massage comes from: four glasses of urine

From the laboratory, not from sexology.

The classic test is named after E.M. Meares and T.A. Stamey, who described it in 1968 in Investigative Urology. The doctor collects four samples: the first stream of urine, the midstream, the fluid that comes from the urethra after a prostate massage, and the urine after that. Which sample the bacteria come from shows where the infection sits. That same massage is therefore described in the clinical monograph of StatPearls as part of the four-glass test and the simpler two-glass version.

What has happened to it since says enough. The Chronic Pelvic Pain guideline of the European Association of Urology from March 2026 calls the four-glass test “too complex for use by practising urologists” and finds that such tests add little to the diagnosis, because 8 percent of patients with suspected prostate pain give a positive culture: about as many as men without complaints. So the medical prostate massage has largely become a historical act. The sexual one is anything but.

Why people enjoy prostate massage

Because with penetration through the anus there is no organ that does more than this.

Daniel R. Dickstein and colleagues write plainly in Nature Reviews Urology that in receptive anal sex the prostate is the primary organ for sexual pleasure. They summarize that with a deep prostate massage twelve pelvic floor contractions have been reported against four to eight with an orgasm through the penis, and that pressure on the prostate, sphincter, and bowel wall can on its own bring on arousal and a climax. Among fifty-three men questioned after prostate cancer treatment, 64 percent said they believe the prostate is a source of sexual pleasure.

What it delivers differs per person.

  • For whoever receives it, the gain often comes down to something new: arousal that does not set off from the penis and that keeps running even without an erection.
  • For whoever does it, the appeal sits in the slowness. You are steering something you cannot force, and that makes it more an exercise in patience than an act.
  • For a couple, it is one of the few things where the roles really turn around without needing a completely different scenario for it.

This does not work for everyone by any means, and the literature is cautious about it too. Dickstein and colleagues call the pleasure from receptive anal sex partly an assumption about how the surrounding nerves take part, not a measured mechanism. So a prostate orgasm is not a button you push.

Misconceptions about prostate massage

“Prostate massage is healthy, so do it regularly.” As a health measure there is nothing behind it. In urology the massage was mainly a way to collect material, and even for that purpose it has, according to the EAU guideline, largely been abandoned.

“With complaints it actually helps.” Exactly the reverse with an acute prostate infection. The StatPearls monograph is brief about that: firm manipulation of the gland must not take place during such an infection, because it can acutely worsen the condition. Fever, chills, or severe pain between the scrotum and the anus are a reason to go to the GP and insert nothing.

“It’s the same as milking.” No. With milking the discharge without a climax is the goal; with prostate massage there is often no goal at all except the feeling itself.

“There’s a button.” There is an area, not a button, and the reaction to it builds up slowly. Whoever searches for a switch presses too hard.

What I see myself

The conversation about it is always harder than the act. The hesitation is about what it means if he enjoys it, not about where the prostate sits.

And the mistake made most often is pace. People set aside ten minutes, notice nothing special happens, and conclude it is not for them. An hour without expectation almost always delivers more than ten minutes with one.

What you can do yourself

  • Start outside before you go in. Pressure on the perineum, the stretch between the scrotum and the anus, already brings on a reaction according to Dickstein and colleagues; that is a calmer starting point than penetrating right away.
  • Use plenty of lube and top it up as you go. The rectum makes none of its own, and working dry is the most common mistake with prostate massage.
  • Cut your nails short or put on a glove. The bowel wall tears more easily than people assume.
  • Skip prostate massage with fever, severe pain, or an ongoing investigation of the prostate, and put the question to a doctor then.
  • Agree that no orgasm has to come. A prostate massage with a duty to deliver a result almost always turns too rough.
